Stroke Program Manager               

Under the supervision of the Director of quality, the stroke program manager administers and coordinates all multidisciplinary activities associated with the stroke program at Lankenau Hospital.  The stroke program manager works in collaboration with the Medical Director of the stroke program, Vice President of Operations and Main Line Hospitals System Quality.  

Essential Accountabilities:

  1.  Collaborates with other members of the interdisciplinary stroke team, the healthcare team, and the care management department to facilitate the delivery and efficiency of quality care for stroke patients.
  2. Provides comprehensive clinical coordination of care and case management through direct care or consultation to influence the plan of care, enhance the contribution of others, and effect change in the system.
  3. Evaluates Stroke/TIA patients for appropriateness of admission type and level of care.  Assists with the coordination of admission for Stroke/TIA patients to the appropriate acute inpatient unit.
  4. Monitors the delivery of care provided to all stroke patients.
  5. If a brain attack occurs during scheduled working hours, immediately respond to Emergency Department to expedite and evaluate the brain attack protocol.
  6. Facilitates the integration of various systems/services including physicians, nurses, therapists, technicians, social workers, and others to ensure an efficient and comprehensive delivery of quality patient care.
  7. Serves as a clinical resource to the interdisciplinary stroke team.
  8. Acts as an expert liaison for nursing.
  9. Works collaboratively with the medical director, emergency department managers and stroke unit managers to define and implement protocols.
  10. Monitors compliance with program standards.
  11. Develops performance measures and reports to the hospital and system stroke committees.
  12. Manages the Get with the Guidelines Stroke data base.
  13. Provides direction to establish, monitor, and evaluate the quality of stroke care for LH.
  14. Actively works with the stroke data abstractor to ensure accurate and timely data collection.
  15. Collates data on quality measures as specified by the stroke manual as well as brain attack measures such as triage time, time to lab results, time to CT scan reading and time to needle with tPA patients.
  16. Coordinates the patient education resources for LH.
  17. Coordinates the education for both internal and external (EMS) team members.
  18. Promotes effective communication with the referring/admitting physicians to encourage support of the stroke program.
  19. Participates in the ongoing development of stroke care through continued research of the most current practice standards as outlined by AHA/ASA.
  20. Develops and implements an educational program for EMS responders in the community on a regular basis.
  21. Develops documentation standards and tools for stroke care.
  22. Collaborates with the emergency department physicians and nursing staff to ensure that the stroke pathways and order sets are appropriately implemented, including timely triage and completion of the diagnostic testing as well as the clinical review for inclusion/exclusion criteria.
  23. Coordinates, attends, and records minutes for the monthly stroke meetings.
  24. Updates the stroke manual as necessary and reviews it annually.
  25. Provides quarterly reports on performance to Joint Commission and monitors stroke care for compliance of standards.
  26. Collaborates with the stroke medical directors to seek and maintain Joint Commission stroke unit certification for LH hospitals.
  27. Works with the Public Relations Department to provide stroke education in the community in the form of health fairs and screenings.
  28. Develops and markets the services of the stroke program to the LH community.

QUALIFICATIONS:

Education:

Graduate of an accredited school of nursing; BSN required. Nurse practitioner preferred.

Licensures & Certifications:

Licensure as a Registered Nurse in the State of Pennsylvania; Nurse Practitioner preferred.      

Experience:

Minimum of 3 years stroke-related clinical experience required; program management, case management, utilization review, and discharge planning experience preferred.

POSITION: Ultrasound Supervisor

JOB SUMMARY:  This position will provide direct supervision of personnel assigned to the ultrasound sections, schedules staff, maintains equipment, orders supplies and maintains logs and records, participates in QA projects.  Must be competent to perform ultrasound services on all patient populations including neonatal, pediatric, adolescent adult and geriatric patients.  Will also recommend hiring, training, personnel movment, scheduling of personnel, and disciplinary action and orient new employees and perform employee evaluations.

Requirements:

  • At least 2 years supervisory experience required, preferably in Ultrasound
  • Three years experience as staff sonographer with experience in ABD/OB required
  • R.D.M.S. in ABD/OB within one year of graduation from applied program required
  • Must have broad exposure in a wide variety of ultrasound examinations including abdominal, OB, neonatal, neuro and vascular procedures. 
  • Must operate numerous pieces of ultrasound equipment.
  • ARRT certification desired

POSITION: Infection Control Practitioner - Lankenau Hospital - Full Time

JOB SUMMARY:  This position will be responsible for developing/implementing a hospital-wide infection control program with an emphasis on performance improvement.  Duties include: enforcement of Standard Precautions; hospital-acquired infection surveillance, specific environmental monitoring; outbreak management; regulatory compliance; administration of educational programs; participation in Quality Improvement initiatives; collection of data and generating reports using Access database, Excel graphs, and spreadsheets; reporting communicable diseases to the local department of health; analyzing infection data for submission to PHC4 for state required reporting system and facilitating meetings of Infection Control Committee.

Requirements:

  • Minimum of 2 years infection control experience preferred. 
  • 3 years medical-surgical nursing experience. 
  • Current Pennsylvania RN license. 
  • BSN preferred, CIC preferred. 
  • Certification in Infection Control (or eligibility). 
  • Attendance at an APIC approved program for Infection Control Practitioners preferred.

POSITION: Clinical Leader OR (General, Urology, Gyn Colorectal)

JOB SUMMARY:
This position, at Lankenau Hospital, will be responsible for the direct and indirect care of perioperative patients. This includes planning, organizing, and directing the specialty services under their charge and communicating pertinent information to provide optimum patient care in the operating room department. Will assist the PCM in assuming responsibility for administrative management of a specific perioperative team and assist in developing and monitoring quality improvement issues. Will also assure that staff is competent in meeting the needs of the patient population on the specialty teams.

  • Current RN state licensure and CNOR within 12 months needed.
  • Bachelor's degree in a related field or currently in progress required.
  • Three years nursing experience in the operating room and current BLS certification also needed.

POSITION: EP/CCL Nurse - Paoli Hospital

JOB SUMMARY:
The EP/CCL Nurse will implement the steps of the nursing process in assessing and rendering physiological and psychological care for patients.  Utilizing knowledge of the principles of growth and development over the life span of these patients, the EP/CCL Nurse will demonstrate the knowledge and skills necessary to provide appropriate care for pre, during, and post procedure EP/Cath patients. The EP/CCL Nurse will identify each patients requirements in accordance with the units and departments approved standards, policies and procedures. The EP/CCL Nurse will be able to assist in Electrophysiology procedures as well as Cardiac Cath Lab procedures. Graduation from an accredited school of professional nursing. A current RN license or temporary practice permit to practice in Pennsylvania. Experience in EP and Cath Lab is required. See unit specific requirements.

Successful completion of all orientation requirements.

Job Summary:  The Patient Care Manager will manage the operations of the nursing unit both clinically and administratively, while demonstrating the knowledge and skills necessary to be accountable for the responsibilities of patient care activities and the development of staff in a fiscally sound manner.  Will work collaboratively with other departments to ensure quality patient care satisfaction.
